Facial lipofilling - what does the procedure involve?
Facial lipofilling is a procedure of transferring the patient’s own fat tissue to restore volume and improve the contour. It allows natural rejuvenation, smoothing of folds and filling of sunken areas of the face.
Unlike synthetic fillers, the transferred fat is the patient’s own tissue - biocompatible, with a natural consistency and a lasting effect. The regenerative cells it contains additionally improve skin quality. We often combine facial lipofilling with a facelift.
Indications for the procedure
Whether the procedure is advisable for you is ultimately decided by the doctor during the consultation.
- Loss of the facial oval, sagging cheeks (jowls)
- Deep nasolabial folds and marionette lines
- Sagging skin of the neck and chin (“turkey neck”)
- Age-related lengthening of the lower face
- A desire for clear rejuvenation without an unnatural pulled effect
- Improvement of the jawline and neck line
Contraindications
How to prepare for the procedure?
The procedure is always preceded by a qualification consultation with a plastic surgeon - we discuss expectations, examine the area to be operated on and establish an individual treatment plan. As standard, we order a full set of tests (blood count, coagulation, electrolytes, ECG for general anaesthesia) and, if necessary, additional specialist consultations.
- Consultation with a plastic surgeon for qualification, discussing expectations and the treatment plan
- Laboratory tests
- Stopping anticoagulant medications at least 14 days before the operation (after consulting a doctor)

Recovery and post-procedure recommendations
- Swelling and bruising are natural and diminish within 2-4 weeks
- Sleeping with the head elevated is recommended for the first few days
- Dressings and sutures cared for according to the doctor’s recommendations
- Sutures are usually removed after 7-14 days
- Avoiding intense physical exertion and bending over for a minimum of 4-6 weeks
- No sunbathing or use of a solarium for a minimum of 6 months - SPF 50 protection is necessary
- After full healing, scar massages and products supporting their maturation may be introduced - according to the doctor’s recommendation
- Check-ups: about 1 month after the procedure, then after 3, 6 and 12 months
Treatment effects
The result of facial lipofilling is restored, youthful fullness of the cheeks and temples, filled tear troughs and shallower folds - the face regains a rested, fresh appearance with no signs of intervention. The part of the graft that takes (60-80%) remains permanently; the effect stabilises after about 3 months.
A facelift significantly improves the jawline and neck contour, reduces skin sagging and deepened folds, giving a younger, fresh yet natural appearance. The effect lasts an average of 8-12 years. Skin ageing will continue, but the face looks much younger than without the procedure. An individual approach allows the scope and technique to be adjusted to achieve the most harmonious result possible.
Possible complications and risks
Like any surgical procedure, this one also carries a certain risk. It is reduced by careful qualification before the procedure and by following the aftercare recommendations. Possible complications include:
- Haematoma or bleeding requiring drainage
- Infection requiring antibiotic therapy
- Temporary or rarely permanent sensory disturbances in the face or auricle
- Hypertrophic or discoloured scars
- Asymmetry requiring correction
- Longer-lasting swelling or bruising
- An unsatisfactory aesthetic result requiring possible re-operation
Frequently asked questions
Does the procedure hurt?
No - we perform facial lipofilling under local anaesthesia with sedation or general anaesthesia, depending on the scope. After the procedure the discomfort is minimal and controlled with medication.
Will the scars be visible?
No - the fat is administered with thin cannulas through 1-2 mm punctures hidden in the natural creases of the skin. The marks are practically invisible.
Where is the fat for the transfer taken from?
Most often from the abdomen, flanks or thighs - with gentle liposuction that also models the donor site. We discuss the donor area at the consultation.
How long does recovery take?
Swelling and any bruising usually subside within 7-14 days; a discreet swelling may persist a little longer.
When can I return to work?
Usually after 5-7 days, once the main swelling subsides; with remote work often earlier.
Is the effect permanent?
Yes - the fat cells that take (usually 50-70% of the administered volume) remain permanently. This is why we sometimes plan a second, complementary stage.
When will I see the final result?
After about 3 months, when the graft stabilises and the swelling subsides. The initial excess volume is intentional - some of the fat is physiologically absorbed.
How does lipofilling differ from hyaluronic acid?
Hyaluronic acid is absorbed within 12-18 months and requires repetition; transferred fat that has taken remains permanently and additionally improves skin quality thanks to the regenerative-cell fraction.
Is the effect of facial lipofilling reversible?
The graft is permanent - which is why we administer the fat conservatively, in small portions. If needed, the volume can be topped up at a subsequent procedure.
Is the procedure suitable for men?
Yes - we perform lipofilling in men with respect for masculine features: a defined jaw angle, chin and flat cheeks.
Can facial lipofilling be combined with other procedures?
Yes - it very often accompanies a facelift, eyelid surgery or chin liposuction, restoring volume where it has been lost.
How to prepare for the procedure?
Tests and qualification, stopping smoking and blood-thinning medications after consulting the doctor. A stable body weight is important - large weight fluctuations change the volume of the graft.

Facial lipofilling: price and consultation
Facial lipofilling costs from PLN 1,500 at Luna Clinic. The price depends on the number of areas, the amount of fat transferred, the anaesthetic and whether it is combined with another operation such as a facelift. Some transferred fat is reabsorbed, so a second session is sometimes planned - we will discuss this at the surgical consultation (PLN 300).
