Upper eyelid surgery

Upper eyelid surgery (upper blepharoplasty) is the most frequently performed procedure for rejuvenating the eye area. It consists in removing excess lax skin (and, if needed, a fat hernia) of the upper eyelid, which droops onto the lashes, narrows the field of vision and gives the face a tired expression.

01
About the treatment

Upper eyelid surgery - what does the procedure involve?

Upper eyelid surgery (upper blepharoplasty) is a surgical procedure consisting in removing excess lax skin and, if needed, fat hernias from the upper eyelid. Its aim is to refresh and rejuvenate the gaze, improve the contour of the eye and remove the feeling of “heavy” eyelids, which may limit the field of vision.

It is a short, outpatient procedure under local anaesthesia - and its effect changes the whole face. The scar is hidden in the natural crease of the eyelid and after healing is invisible even with the eyes closed.

02
Indications

Indications for the procedure

Whether the procedure is advisable for you is ultimately decided by the doctor during the consultation.

  • Excess lax skin of the upper eyelid (a so-called drooping eyelid)
  • A skin fold limiting the field of vision
  • Fat hernias causing a puffy or “bag” effect
  • A tired, sad appearance of the gaze
  • Functional and aesthetic discomfort
03

Contraindications

Uncompensated chronic diseases (diabetes, hypertension)
Blood clotting disorders
Active skin infections in the eyelid area
Unstable eye diseases (e.g. severe dry eye syndrome)
Unrealistic expectations regarding the result
04
Preparation

How to prepare for the procedure?

The procedure is always preceded by a qualification consultation with a plastic surgeon - we discuss expectations, examine the area to be operated on and establish an individual treatment plan. As standard, we order a full set of tests (blood count, coagulation, electrolytes, ECG for general anaesthesia) and, if necessary, additional specialist consultations.

  • Consultation with a plastic surgeon
  • Stopping anticoagulant medications at least 14 days before the procedure (after consultation)
  • Avoiding alcohol and smoking at least 14 days before the procedure
  • On the day of the operation the skin should be clean, without make-up
05

The upper-eyelid-surgery procedure step by step

1Qualification and measurement of excess skinthe surgeon precisely determines the amount of skin to be removed - so that the eyelid closes freely and the effect is natural.
2Local anaesthesiathe procedure on both eyelids takes about 45-60 minutes; the patient stays in contact with the team.
3Incision in the eyelid creasethe incision line runs in the natural crease - the scar will remain hidden in it.
4Removal of excess skin and fata spindle-shaped piece of skin is excised; if needed, the medial fat hernia is reduced.
5Intradermal suturingthe wound is closed with a very fine suture, removed after 5-7 days; the patient goes home immediately.
06
Recovery

Recovery and post-procedure recommendations

  • Swelling and bruising usually subside in 7-14 days
  • The sutures are removed after 5-7 days
  • The scars should be protected from the sun for a minimum of 6 months
  • We recommend cool compresses in the first days after the procedure
  • Avoiding physical exertion for about 2 weeks
  • Using products supporting scar maturation after complete healing

Treatment effects

The result of upper eyelid surgery is an open, rested gaze - the eyelid regains a visible crease, the feeling of heaviness disappears, and in some patients the field of vision also improves. The effect lasts 8-12 years. It is a procedure with one of the best “low invasiveness - big change” ratios.

The result is a refreshed, younger gaze, reduced excess skin and a better eyelid contour. The result is permanent, but the skin-ageing process will continue naturally. The final effect can be assessed after about 2-3 months, when the swelling disappears and the scar heals.

Possible complications and risks

Like any surgical procedure, this one also carries a certain risk - at Luna Clinic we minimise it through careful qualification, operating in operating-block conditions and the team’s experience. Possible complications include:

  • Haematoma or bleeding requiring drainage
  • Infection requiring antibiotic therapy
  • Temporary or rarely permanent sensory disturbances in the operated area
  • Hypertrophic or discoloured scars - minimised thanks to precise suturing technique
  • Asymmetry requiring correction
  • Longer-lasting swelling or bruising
  • An unsatisfactory aesthetic result requiring possible re-operation

Frequently asked questions

Is the procedure painful?

Local anaesthesia effectively eliminates pain during the procedure. After the operation slight discomfort is possible, relieved with painkillers.

Will there be a scar?

Yes - the scar is thin and hidden in the natural crease of the eyelid, usually barely visible after healing.

When can I return to work?

Most patients return to office work after about 7-10 days.

Is the effect permanent?

The effect lasts many years, although the skin-ageing process continues naturally.

Can the procedure be repeated?

Yes, if needed the procedure can be performed again after years.

When can I wear make-up?

Usually after 10-14 days, once the wounds have healed and the sutures are removed.

Can it be done under general anaesthesia?

We perform it as standard under local anaesthesia, but at the patient’s request - under general.

Why don’t we combine it with the lower eyelids?

The lower eyelids often look better after upper eyelid surgery alone, because the fatty infiltration from above decreases. We plan the correction of the lower ones in stages - the decision is usually made about a month after the upper operation, if the problem remains.

When can I return to sports?

Light activity after about 2-3 weeks, intense sport after consultation with the surgeon.

Can I drive a car?

Yes - usually from the next day, if there is no swelling significantly limiting vision.

Can upper and lower eyelid surgery be performed together?

Yes - it is a common combination, performed under a single anaesthesia, giving full rejuvenation of the eye area.

When can I return to make-up and contact lenses?

Eyelid make-up after about 2 weeks (once the wound has fully healed), contact lenses usually after 7-10 days.
