Lower eyelid surgery - what does the procedure involve?
Lower eyelid surgery (lower blepharoplasty) is a surgical procedure aimed at removing excess skin and fat hernias from under the lower eyelid. It improves the appearance of “under-eye bags” and smooths the lower eyelid area, restoring a younger and more rested appearance.
Depending on the problem, we use a transconjunctival approach (from inside the eyelid - with no visible scar, when only fat hernias are the issue) or a transcutaneous one just below the lash line (when excess skin must also be removed). We increasingly reposition the fat instead of removing it - using it to fill the tear trough.

Contraindications
How to prepare for the procedure?

As standard, we order a full set of tests (blood count, coagulation, electrolytes, ECG for general anaesthesia) and, if necessary, additional specialist consultations.

Recovery and post-procedure recommendations
- Swelling and bruising are natural and diminish within 2-4 weeks
- Sleeping with the head elevated is recommended for the first few days
- Dressings and sutures cared for according to the doctor’s recommendations
- Sutures are usually removed after 7-14 days
- Avoiding intense physical exertion and bending over for a minimum of 4-6 weeks
- No sunbathing or use of a solarium for a minimum of 6 months - SPF 50 protection is necessary
- After full healing, scar massages and products supporting their maturation may be introduced - according to the doctor’s recommendation
- Check-ups: about 1 month after the procedure, then after 3, 6 and 12 months

Is the procedure painful?
No - performed under anaesthesia. After the procedure the pain is controlled with medication.
Will there be scars?
Yes, but placed just below the lashes or on the conjunctival side - usually practically invisible.
When will I see the final result?
Most often after 4-6 weeks, when the swelling disappears; complete healing takes up to 7 months.
How long does recovery take?
Most patients can return to work after about 7-10 days. However, it is worth remembering that for a few weeks the lower eyelid may still be slightly tense or seem pulled away from the eyeball - this is a normal stage of healing. The tightening of the tissues and the return of the eyelid to its natural position can take a few weeks, so we usually assess the final effect after about 2-3 months.
Why don’t you combine the lower and upper eyelids at the same time?
Because upper eyelid surgery often already partly solves the problem of the lower ones - a staged approach helps avoid overcorrection and complications.
Can I drive after the procedure?
Yes - once the effect of the sedation has worn off and the swelling allows free vision.
Is the effect permanent?
Yes - although the ageing process continues, the effect lasts many years.
Can it be combined with other procedures?
Yes - e.g. with a facelift, but we do not combine it simultaneously with upper eyelid surgery.
How to prepare for the procedure?
Consultation, laboratory tests, stopping medications as recommended, avoiding stimulants.
Does the procedure require a stay in the clinic?
No - the patient goes home the same day.
Does lower eyelid surgery remove under-eye shadows?
It removes shadows resulting from fat bulging and collapse of the tear trough. Shadows of a vascular or pigmentary origin require additional other methods - we will assess this at the consultation.
Is there a visible scar after the procedure?
With the transconjunctival approach there is no external scar; with the transcutaneous one the scar just below the lashes is practically invisible after healing.
