Mastektomia
Mastectomy (so-called top surgery) is an operation to surgically remove breast gland tissue together with excess skin and to shape a flat, masculine chest. For transgender (FtM) and non-binary people it is often the most important stage of gender affirmation - bringing a huge improvement in psychological comfort and daily functioning (an end to binding).

Mastectomy - what does the procedure involve?
Mastectomy (so-called top surgery) is an operation consisting in the surgical removal of breast gland tissue and the appropriate shaping of a masculine chest contour. At Luna Clinic we treat this procedure as an important element of gender affirmation and support for FtM or non-binary transgender people. In our practice, qualification for the procedure is possible only for people after a legal change of the registered gender marker, which ensures compliance with legal requirements and the safety of the medical process. The technique is selected individually, taking into account the patient’s anatomy and expectations.
We choose the technique according to breast size and skin elasticity: the periareolar method (for small breasts - minimal scars) or the double-incision method with a free nipple graft, giving full control over the contour for larger breasts. The removed tissue is sent for histopathological examination.
Indications for the procedure
Whether the procedure is advisable for you is ultimately decided by the doctor during the consultation.
- Gender dysphoria related to the presence of breasts
- A wish to achieve a masculine or neutral chest contour
- Ineffective or insufficient breast reduction on HRT
- Improvement of quality of life and psychological comfort
- A need to make daily life easier (e.g. no need to wear binders)
Contraindications
How to prepare for the procedure?
The procedure is always preceded by a qualification consultation with a plastic surgeon - we discuss expectations, examine the area to be operated on and establish an individual treatment plan. As standard, we order a full set of tests (blood count, coagulation, electrolytes, ECG for general anaesthesia) and, if necessary, additional specialist consultations.
- Consultation with a plastic surgeon experienced in transgender surgery
- Anaesthesiological consultation
- Laboratory tests
- Documentation confirming the diagnosis of gender dysphoria
- Stopping aspirin, anticoagulants and certain supplements at least 14 days before the procedure
- No smoking or alcohol for at least 2 weeks before the procedure
- Maintaining a stable body weight (best results at BMI <30)
- A thorough discussion of expectations regarding the shape of the scars and nipples
The mastectomy (top surgery) procedure step by step
Recovery and post-procedure recommendations
- Stay in the clinic: about 12 h
- Wearing a compression vest for at least 8 weeks
- Limiting physical exertion for a minimum of 4-6 weeks
- No lifting of heavy objects or raising the arms above shoulder level for 4 weeks
- Avoiding sun exposure of the scars for at least 6 months
- Starting massage and scar products after complete healing (usually after 3-4 weeks)
- Check-ups: after 1, 3, 6 and 12 months
Treatment effects
The result of mastectomy is a flat, masculine chest with nipples of masculine size and position. The scars (with double incision running horizontally in the shadow of the pectoral muscle) mature and fade within 12 months. For the vast majority of patients the procedure means a step change in quality of life.
Possible complications and risks
Like any surgical procedure, this one also carries a certain risk - at Luna Clinic we minimise it through careful qualification, operating in operating-block conditions and the team’s experience. Possible complications include:
- Haematoma or bleeding requiring drainage
- Infection requiring antibiotic therapy
- Temporary or rarely permanent sensory disturbances in the operated area
- Hypertrophic or discoloured scars - minimised thanks to precise suturing technique
- Asymmetry requiring correction
- Longer-lasting swelling or bruising
- An unsatisfactory aesthetic result requiring possible re-operation
Frequently asked questions
Czy zabieg boli?
It is felt mainly in the first days, but the pain is controlled effectively with medication.
Do I have to be on hormones to have the procedure?
No, but for many people hormone therapy makes it easier to achieve the desired results.
Are there always scars?
Yes - their length and position depend on the chosen technique. We work so that they are as inconspicuous as possible.
How long does the return to work take?
Office work - usually after 7-10 days. Physical work - after 4-6 weeks.
Can I choose the shape and position of the nipples myself?
We determine the range of possibilities together at the consultation.
Will there be drains?
Yes - you go home without them.
Is the effect permanent?
Yes - the removed gland tissue does not grow back.
Are further operations needed?
Usually not - unless the patient wants to improve the result or change the shape of the nipples.
How long is the compression vest worn?
We recommend a minimum of 8 weeks.
Can I combine the procedure with other procedures?
Sometimes yes - e.g. liposuction of the abdomen or flanks - we decide this individually.
What conditions must be met before gender-affirming mastectomy?
Appropriate documentation is required (specialists’ opinions in accordance with current standards of care). We discuss the details and the full list individually at the consultation - we help you go through the formalities.
Does sensation in the nipples return after the procedure?
With a free nipple graft, sensation is usually weakened or permanently altered; pedicled techniques preserve it better. We discuss the choice of method openly, weighing the patient’s priorities.
Can I train my chest after mastectomy?
Yes - after full healing (about 6-8 weeks) chest training is advisable; developing the pectoral muscle further improves the masculine contour.

What does the mastectomy (top surgery) involve?
Mastectomy is a procedure in the field of plastic surgery. At Luna Clinic, every treatment plan is created individually - we start with a thorough consultation and qualification, during which the doctor discusses expectations, assesses the starting situation and selects the scope and technique of the procedure. Thanks to this, the mastectomy (top surgery) is safe, predictable and tailored to the anatomy and real needs of the specific person. We focus on a natural rather than artificial effect - so that the result emphasises individual beauty and remains in harmony with the rest of the figure or facial features. You will find a full, detailed description of the method in the upper part of this page.
the mastectomy (top surgery) - who is it for? Indications and qualification
The best way to answer whether the mastectomy (top surgery) is advisable in your case is during a consultation. The doctor always decides on qualification after an examination - we take into account the state of health, expectations and the real possibilities of the given method. If the procedure is not needed or safe at the moment, we will say so directly and suggest a sensible alternative. Such an honest conversation is the basis of a good decision and satisfaction with the result - which is why we never persuade anyone into unnecessary procedures.
How does the procedure go?
The course is planned to take maximum care of your comfort and safety. We work in comfortable conditions that meet all standards, and an experienced team oversees the process. All recommendations - preparation, contraindications and post-procedure conduct - are discussed individually and also provided in writing, so that the mastectomy (top surgery) proceeds calmly, predictably and without surprises.
Effects and durability - the mastectomy (top surgery) before and after
The effects that the mastectomy (top surgery) provides are always assessed individually. A natural, harmonious refreshment of appearance or an improvement of proportions is the goal we strive for with every patient. “Before and after” transformations are best viewed in our results gallery - they show the real outcomes of our work. Remember that the durability and quality of the effect are also influenced by lifestyle, care and consistent adherence to post-procedure recommendations.
Recovery and post-procedure recommendations
Recovery goes most smoothly when you follow the recommendations closely. After the procedure you remain under the clinic’s care - in the first days, contact with a doctor is available, and we schedule follow-up visits until full healing and assessment of the effect. You receive clear instructions on care, activity and any supporting products in writing, so you know exactly what to expect at each stage of returning to form.
Safety, experience and trust
Safety is an absolute priority for us. Mastectomy is performed by Dr Aleksandra Łuniewska - a plastic surgery specialist, with care for safety and an individual approach to the patient. Careful qualification, work in appropriate conditions and the team’s experience allow us to minimise risk and achieve repeatable, aesthetic results.
