Fleur-de-lis abdominoplasty
Fleur-de-lis abdominoplasty is an extended variant of the tummy tuck, in which excess skin is removed both horizontally and vertically - the incisions form the shape of an inverted T (a French lily, hence the name). It is the treatment of choice for patients after very large weight loss, in whom a classic abdominoplasty would not remove the excess skin in the midline and upper abdomen.

Fleur-de-lis abdominoplasty - na czym polega zabieg?
The operation is performed under general anaesthesia and usually takes 3-4 hours.
The surgeon makes a horizontal incision in the lower abdomen and an additional vertical incision from the sternum to the pubic symphysis.
Excess skin is removed in both directions, the rectus abdominis muscles are sutured, and the navel is moved to an anatomical position.
Drains and a compression dressing are applied. The stay in the clinic is usually 1 night.
The fleur-de-lis technique allows maximum narrowing of the waist and tightening of the abdominal wall also in the transverse dimension - at the cost of an additional vertical scar in the midline of the abdomen. In bariatric patients this “skin for a scar” trade-off gives a decidedly better body contour.
Indications for the procedure
Whether the procedure is advisable for you is ultimately decided by the doctor during the consultation.
- Significant excess skin after massive weight reduction (>40-50 kg)
- Sagging of tissues in the vertical and horizontal axis
- Extensive separation of the rectus abdominis muscles
- Deformation of the torso contour with skin folds on the midline side
- Hygiene difficulties, chafing, recurrent inflammation in the skin folds
- A lack of results after a classic abdominoplasty
Contraindications
How to prepare for the procedure?
The procedure is always preceded by a qualification consultation with a plastic surgeon - we discuss expectations, examine the area to be operated on and establish an individual treatment plan. As standard, we order a full set of tests (blood count, coagulation, electrolytes, ECG for general anaesthesia) and, if necessary, additional specialist consultations.
- Laboratory tests
- Anaesthesiological consultation
- Stopping medications that affect coagulation
- No smoking or alcohol for a minimum of 14 days
- On the day of the procedure the abdomen should be washed, without lotions or cosmetic products.
The fleur-de-lis abdominoplasty procedure step by step
Recovery and post-procedure recommendations
- A bent (V) position for 2 weeks, limited physical activity
- Mandatory wearing of a compression band for a minimum of 6-8 weeks
- No lifting, intense exercise or sun exposure for a minimum of 6 weeks
- Scar massage and the use of products supporting healing after about 4 weeks
- Check-ups after 7-10 days, then after 1, 3, 6 and 12 months
- The option of choosing compression underwear at the clinic after healing
Treatment effects
The result of fleur-de-lis abdominoplasty is a radical reduction of excess skin, a flat abdomen and a clearly narrowed waist - a result unattainable with the classic technique in patients after massive weight reduction. The scars (horizontal and vertical) mature and fade over 6-12 months.
The procedure achieves an aesthetic, taut and flat abdomen. The waistline becomes clearer, and the muscle separation is closed.
The effect is permanent, provided the patient maintains a stable weight and is not planning another pregnancy.
The scar is made low - below the underwear line - and over time becomes thin and light.
Possible complications and risks
Like any surgical procedure, this one also carries a certain risk - at Luna Clinic we minimise it through careful qualification, operating in operating-block conditions and the team’s experience. Possible complications include:
- Haematoma, infection, necrosis of the wound edges
- Asymmetry or deformation of the navel
- Long-lasting swelling, sensory disturbances
- Seroma, venous thrombosis (hospital prophylaxis)
- A hypertrophic scar or one difficult to hide in the midline (the choice of incision technique is crucial)
Frequently asked questions
Who is this procedure for?
For people with a large excess of skin - mainly after massive weight loss or bariatric treatment.
How does it differ from a classic tummy tuck?
The additional vertical incision allows removal of excess skin in the vertical axis - impossible to achieve with a classic abdominoplasty.
Is the scar very visible?
The inverted-T-shaped scar is larger than in the classic operation but necessary to achieve the correct result.
How long does recovery take?
To office work usually after 3-4 weeks. To full physical activity - after 6-8 weeks.
Can the procedure be combined with thigh or breast surgery?
Sometimes yes - but we most often recommend a staged approach.
When will I see the result?
The first effects are visible after a few weeks. For the final result you have to wait 3-6 months.
Is the muscle separation sutured?
Yes - during the procedure we suture the linea alba if diastasis recti is present.
Does the procedure hurt?
Post-operative pain is effectively controlled with medication.
Is the effect permanent?
Yes - with a stable body weight maintained.
Can this procedure improve my posture?
Yes - suturing the muscles can improve torso stabilisation and posture.
Is the vertical scar after fleur-de-lis very visible?
Initially yes, but with proper care it fades within a year. Patients after major weight loss almost always regard this trade-off as beneficial relative to the removal of skin folds.
When is fleur-de-lis chosen instead of a classic tummy tuck?
When the excess skin occurs not only in the vertical axis (a suprapubic fold) but also the transverse one - usually after losing several dozen kilograms.

What does fleur-de-lis abdominoplasty involve?
Fleur-de-lis abdominoplasty is a procedure in the field of plastic surgery. At Luna Clinic, every treatment plan is created individually - we start with a thorough consultation and qualification, during which the doctor discusses expectations, assesses the starting situation and selects the scope and technique of the procedure. Thanks to this, fleur-de-lis abdominoplasty is safe, predictable and tailored to the anatomy and real needs of the specific person. We focus on a natural rather than artificial effect - so that the result emphasises individual beauty and remains in harmony with the rest of the figure or facial features. You will find a full, detailed description of the method in the upper part of this page.
fleur-de-lis abdominoplasty - who is it for? Indications and qualification
The best way to answer whether fleur-de-lis abdominoplasty is advisable in your case is during a consultation. The doctor always decides on qualification after an examination - we take into account the state of health, expectations and the real possibilities of the given method. If the procedure is not needed or safe at the moment, we will say so directly and suggest a sensible alternative. Such an honest conversation is the basis of a good decision and satisfaction with the result - which is why we never persuade anyone into unnecessary procedures.
How does the procedure go?
The course is planned to take maximum care of your comfort and safety. We work in comfortable conditions that meet all standards, and an experienced team oversees the process. All recommendations - preparation, contraindications and post-procedure conduct - are discussed individually and also provided in writing, so that fleur-de-lis abdominoplasty proceeds calmly, predictably and without surprises.
Effects and durability - fleur-de-lis abdominoplasty before and after
The effects that fleur-de-lis abdominoplasty provides are always assessed individually. A natural, harmonious refreshment of appearance or an improvement of proportions is the goal we strive for with every patient. “Before and after” transformations are best viewed in our results gallery - they show the real outcomes of our work. Remember that the durability and quality of the effect are also influenced by lifestyle, care and consistent adherence to post-procedure recommendations.
Recovery and post-procedure recommendations
Recovery goes most smoothly when you follow the recommendations closely. After the procedure you remain under the clinic’s care - in the first days, contact with a doctor is available, and we schedule follow-up visits until full healing and assessment of the effect. You receive clear instructions on care, activity and any supporting products in writing, so you know exactly what to expect at each stage of returning to form.
